The title is good, but if you would like to request a convert please create the request as any normal item requests then add the personal comment to the Disqus section below the request.
1, Please only add the item description next time from the store page, just CTRL+C CTRL+V the body text from the details/description, and add the personal bits as a comment at the bottom of the request. If someone Fills your request and auto-posts it to the blog, your comment in the description would carry over and it would confuse the others.
